Responsibilities Temecula Valley Hospital Temecula Valley

please visit Administrative Coordinator - UR/Case Management The
Administrative Coordinator for the Case Management Team is
responsible for coordinating the day to day administrative needs of
the Case Management team. Essential Duties: The Admin Coordinator
will be responsible for answering calls, taking messages and
distributing to the appropriate Case Manager/Social Worker. This
includes filing and maintaining discharged patients paperwork and
updating the discharge planning section of the patient's original
chart. The Coordinator ensures that all supplies needed to maintain
the office are reviewed and stocked daily as needed. Retrieves
departmental mail daily and delivers to Manager/Director. Is
responsible for maintaining both the Care Coordinator and Social
Service resources phone list. Additional responsibilities will
include; prints census, admission/discharge/transfer reports daily.
Distributes care coordinator assignments to units, insurance
verifiers and House Supervisor. Distributes referrals to
appropriate care coordinator for follow-up. Retrieves copy of
Physician order and attaches to referral prior to distribution to
Care Coordinators. Assists with making calls to vendors to procure
services for patients being discharged (home health, durable
medical equipment, skilled nursing facilities etc.). Faxes
appropriate patient information to vendors. Qualifications Minimum
1 year clerical assistant experience; clinical clerical experience
preferred Previous hospital experience preferred Minimum High
