Job Description:
A company is looking for a Remote Case Manager RN.
Key Responsibilities:
Develop and implement personalized care management plans
Assess and prioritize member care needs during initial
interactions
Educate members and families to enhance treatment adherence
Qualifications:
Nursing Degree from an accredited school (BSN preferred)
Minimum 2 years of clinical experience; care management in health
insurance is preferred
CCM Certification (to be achieved within two years of hire)
Unrestricted RN license, with a compact or Washington license
required
