Job Description:
A company is looking for a NICU Case Manager, LCSW (Remote).
Key Responsibilities:
Empower caregivers and families to support optimal wellness and
advocate for necessary services and community resources
Assess, plan, and implement individualized care strategies directed
toward appropriate levels of care
Build relationships with caregivers and healthcare providers to
develop member-focused care plans addressing physical and
psycho-social barriers
Required Qualifications:
Minimum of 2 years of experience in Pediatrics/NICU managed care or
clinical environment
Valid unrestricted Social Worker license in the state of residence,
with willingness to obtain licensure in other states as needed
Five or more years of managed care experience in a case management
role for 100% telecommuting positions
Experience with a Medicaid population is preferred
CCM certification preferred upon hire, with the requirement to
obtain it within specified timeframe
