General Statement of Position Provides nursing care to residents in Memory Care community's through assessment, planning implementations and reevaluation of resident needs. Carries out and interprets policies, procedures, local, state, and federal regulations. Provides person-centered physical and psychological support, care, and service to all residents and their families. Provides leadership to CNAs, and Resident Assistants. Essential Functions The following duties are normal for this position. These are not to be construed as exclusive or all-inclusive. Other duties may be required and assigned. * Assesses resident needs, and provides necessary nursing care as per resident's care plan. * Oversees CNAs and Resident Assistants. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include training employees; planning, assigning, and directing work; appraising performance, addressing complaints and resolving problems. * Ensures that resident rights are maintained * Administers prescribed medications and treatments in accordance with approved nursing techniques and per physician orders. * Documents nursing history and physical assessment for assigned patients upon admission; * Handles resident, family, or physician complaints/concerns, referring issues to CSD as necessary. * Respects advance directives and assists residents to a dignified death; and provides emotional and spiritual support to family and other staff as needed. * Responds to life threatening situations based upon nursing standards, policies, procedures, and protocol. * Participates in department and/or unit performance improvement activities. * Other duties as assigned * Attends in-service training programs for department specific and other relevant programs, i.e., safety, infection control, orientation, etc., as scheduled. * Is aware of and carries out general community and specific departmental safety policies; always reports hazards/safety issues promptly. * Is aware of and practices principles of infection control; reports infection control issues promptly. * Excellent leadership and management of the care team that honors, respects and values individual opinions and suggestions, demonstrates fair treatment and ethical business practices, supports development and rewards achievement. * Manages electronic reporting system, occurrence and situational reporting and Medication Administration Records * Communicates with physicians, families and management as appropriate on resident's change of condition or incident * Assists with resident and staff emergencies as permitted by regulation and in compliance with Advanced Directives Minimum Training and Experience * Prior experience as a shift nurse * Minimum of three years of experience in Assisted Living, preferably with residents with Alzheimer's disease or other types of dementia * A licensed healthcare professional - LVN/LPN/RN or equivalent * Knowledgeable of responsibilities pertaining to Elder Abuse, including all aspects of reporting * Strong knowledge of current RCFE regulations; RCFE certification highly desirable * Completion of First Aid, CPR, Blood borne Pathogen and other mandated training within first thirty days of employment and prior to resident contact * Legal Right to work in the USA (we E-Verify) * Able to successfully pass a DOJ/FBI background check (LiveScan), a pre-employment physical exam and be free of Tuberculosis as evidenced by a TB test. (No DUI's please). * Must be able to obtain first aid/CPR certification within 30 days of hire; * Excellent communication skills (verbal and written) in English. * Experience with computerized record management, medication management and other appropriate computer applications * Exceptional interpersonal skills in dealing with staff, residents, families, health care professionals, Licensing, Ombudsman, and others; * Ability to work a flexible schedule to meet the community needs, with regular and timely attendance required. * Ability to life up to 60 lbs., push up to 40 lbs., and carry up to 30 lbs.
